Lhutel the Headless

Can be found in Tomsword Catacombs west of the Minor Erdtree in Weeping Peninsula (Limgrave region). Defeat the boss, and you will get it.

Redmane Knight Ogha

Defeat boss inside the War-Dead Catacombs, which you can find north of Radahn boss fight arena. It's north on the huge area where you end up fighting with Radahn.

Black Knife Tiche

Complete "Ringleader’s Evergaol" in the northwest corner of Moonlight Altar (this is the southwestern plateau of Liurnia region). You will need to finish Lake of Rot, which you enter from Nokstella after the boss fight there. At the bottom of the Lake (Grand Cloister site of grace) there is a pathway which at the end has a coffin - enter it to teleport to Moonlight Altar.

Mimic Tear

Mimic Tear is in Nekron - underground area that unlocks after beating Radahn boss. You can check my game progress guide topic and see all of the steps how to get towards that fight. After you enter Nekron, you will have to find it, it's after Ancestral Woods site of grace, beyond the Stonesword Key Doors when you fall down into the city and reach the room with altar after the mentioned site of grace.

Ancient Dragon Knight Kristoff

Defeat boss inside Saint Hero's Grave dungeon in Altus Plateau.

Cleanrot Knight Finlay

This one is Elphael, teleport to "Prayer Room" Site of Grace and just go straight, down the first set of stairs, and jump over to the balcony which has candles. It's inside the next room.
