Radagon’s Soreseal

You can find it in Fort Faroth in Caelid Region. Enter the tower, climb the ladder, jumb on lower platforms which will get you on top of the tower, make your way into the wooden platform with ladders from the roof, go left, look back, jump to another otherwise unreachable wooden platform, fall down, get the Soreseal.

Found in a chest in Nokstella under the big throne.

Dragoncrest Greatshield Talisman

Teleport to "Drainage Channel" Site of Grace in Elphael, run on the roots and jump through the hole in the roof to get into the chappel. Check the balcony to find a chest.

Marika’s Soreseal

Teleport to "Drainage Channel" Site of Grace in Elphael, go outside and keep dropping down until you reach the bottom. Open a Stonesword Key Doors and find it at altar inside.

Old Lord’s Talisman

In Farum, teleport to "Beside the Great Bridge" Site of Grace and follow the path different from the bridge towards the boss (so, go left, and reach the end of the path where you will be invaded by Recusant Bernahl and attacked by 3 more enemies. There is also a chest there.

Erdtree’s Favor +2

After the Royal Capital becomes a desert near the end of the game, from the Forbidden Lands east of the capital, go back towards the Royal Capital, take the elevator down and you will find it on a tree. It's accessible when you complete Crumbling Farum Azula.
